Before applying, make sure you meet the following qualifications:
- 18 years of age or older, or a pediatric patient with parental consent
- Diagnosed with a qualifying medical condition – the list of adult and pediatric conditions can be found here
- You have a Physician Certification from an MD or DO licensed in Delaware (Read “Talking With Your Doctor”)
- A Delaware resident with proof of residency (DE Driver’s License or DE State ID)
Applying for your medical marijuana card is simple: Do it online here or print and mail the completed application. After your doctor authorizes treatment and the State receives the application, it could take two to four weeks for you to receive your card. Once you get your card, note that we give all new patients a 20% discount on their first purchase of medicated products.
Also, beginning October 1, for new patients spending $100 or more on their second visit, FSC is providing a one-time-only $50 discount. This is being done to offset the $50 fee charged by the Medical Marijuana Program for medical marijuana cards.
Your new card is valid for one year from date of issuance. The process to renew your card is basically the same and uses the same form. You will still need to have your application signed by a physician and verified by the State, and you must pay a State fee.
If you’re homebound or need assistance, you can designate a caregiver to purchase the medication on your behalf. Caregivers must be approved by the State through an application process. Download the application here to fill out and mail in.
